HARRY’S COMMENTS: We are working with a confidential client in their search for a Performance Growth Marketing, Affiliates Lead. The company is a national, direct-to-consumer telehealth and telepharmacy platform, one of the original pioneers in the category.

They connect patients with licensed U.S. providers for the conditions you’d expect any modern telehealth brand to treat: weight loss and GLP-1, mental health, men’s sexual health, women’s health, hair loss, skin, and a growing list of chronic conditions managed by Rx subscription.

Their model is cash-pay only. No insurance. Three steps: online assessment, U.S.-based clinician review, free medication delivered to the patient’s door in two to three business days.

Unlike most of their competitors, this client owns the pharmacy. They employ their own doctors. They compound their own drugs. They run their own fulfillment. That is a real margin advantage, and it is the lever the new Affiliates Lead gets to pull when negotiating publisher payouts, performance-celebrity deals, and tier-1 commerce-content placements.

DTC Telehealth: Crowded, but there’s plenty of room to compete.

Last year the U.S. telehealth market was valued at roughly $78 billion, and the most credible forecasts put it on a 10% to 24% CAGR through 2030. North America holds about 54% of global share. There is more than enough oxygen in this category for a vertically integrated, lean, well-positioned brand to win meaningful share, particularly one that competes on medical legitimacy and patient care rather than lifestyle branding.

Who is this client’s real customer?

About who you’d expect: a 25- to 55-year-old, college-educated, household income $50K+, digitally fluent, time-poor, and privacy-conscious. They’ve often tried the traditional system and found it frustrating. They want their birth control refill, their SSRI, their GLP-1, their ED medication, or their cholesterol Rx without the two-hour office visit and the awkwardness.

Women skew higher on mental health, weight loss, birth control, and UTIs. Men dominate ED and hair loss. The unifying psychological thread: they want to feel better about themselves, and they don’t trust the traditional system to make that easy.

Why should a proven affiliate pro want this role? Three reasons …

First, this is a true greenfield: There is no real program today, no deep partner roster, no onboarding muscle, and no community presence on Reddit or Quora. You are not inheriting somebody else’s mess; you are building.

Second, the client has an operator-led mandate. The marketing org is lean, roughly a dozen at the entity level, which means direct access to the President and to the CMO, no layers of middle management, and a base + KPI-tied bonus structure that lets you share in the program you build.

Third, this category rewards real affiliate craft. Telehealth affiliate is the most regulated, most fraud-prone, most compliance-sensitive flavor of affiliate marketing that exists. TCPA, HIPAA, LegitScript, FTC endorsement guides, FDA promotional rules: the bar is high. If you’ve already navigated this at Hims, Ro, 30 Madison / Remedy Meds, Nurx, LifeMD, Cerebral, or a comparable cash-pay DTC subscription, you will run circles around the field.

You’ll work for a President with an operator background, someone who has run a multi-hundred-million-dollar consumer P&L at a prior role and who has been hands-on running this business since the acquisition closed earlier this year. He’s already grown the company roughly 30% in the months since.

He understands that affiliate is “more art than science” and is bringing in subject-matter expertise precisely because his marketing team is full of generalists and this channel needs a specialist who lives and breathes it.

ABOUT THE ROLE:

You will own two high-upside acquisition channels: the company’s affiliate program and community-driven growth across Reddit, Quora, and similar Q&A forums where patients are actively researching treatments.

Your mission will be to …

1.) build a clean, scalable, fraud-resistant affiliate program from the ground up using EverFlow (or your preferred equivalent),

2.) recruit and develop a roster of publisher, influencer-affiliate, and media-buying partners who drive measurably incremental patient acquisition, and

3.) build the company’s first authentic community presence on Reddit and Quora that converts research-stage patients into consultations.

The platform is already at scale, the pharmacy infrastructure is in place, and the recent acquisition has restored operating focus. There are no direct reports to start, and you are expected to operate as a hands-on-keyboard subject matter expert who builds the function, not delegates it.

You won’t believe how much we know about this search …

I spent a ton of time on the phone with this client teasing out exactly what you’ll need to KNOW and DO to hit the ground running in this role. Some of the things I can share with qualified candidates include …

KEY RESPONSIBILITIES:

  • Build and run the affiliate program end-to-end inside EverFlow (or a comparable platform), including publisher and partner recruitment, vetting, contract negotiation, payout structure design, gamified milestone bonuses, attribution-window calibration, and ongoing program hygiene.
  • Recruit a layered partner mix across content-driven publishers (tier-1 outlets like Healthline, Hearst-owned commerce-content properties, Condé Nast verticals, and Good Housekeeping), tier-2 niche health publishers, influencer-affiliates posting on Instagram Reels and TikTok with affiliate links, and selectively-vetted media-buying affiliates running CPA-model traffic on platforms where category compliance allows.
  • Negotiate every flavor of deal the channel uses today: flat-fee accelerants, CPA, CPC, revenue share, hybrid flat + commission with influencers, and performance-based celebrity deals where credible advocates work on a CPA basis instead of a flat brand-deal fee.
  • Stand up Reddit and Quora as patient-acquisition channels by identifying high-intent communities (weight loss, mental health, men’s sexual health, women’s health, GLP-1, ED, hair loss), developing authentic engagement strategies that respect community culture, and building the brand’s presence over time without triggering bans.
  • Monitor program quality and run continuous fraud detection: deploy leak-proof one-time-use codes, geographic traffic analysis, click-to-conversion velocity tracking, brand-bid auditing via VPN, cookie-stuffing detection, and integrations with the industry-standard fraud tools (24metrics, IPQualityScore, Anura). Industry data says 5% to 20% of affiliate commissions in a typical DTC program are fraudulent. Find them, claw them back, and prune the affiliates who generate them.
  • Design and test landing pages and offer structures specific to affiliate and community traffic sources, including front-end self-liquidating offers calibrated to the company’s true contribution margin per patient and per condition.
  • Build measurement infrastructure that goes beyond platform-reported, last-click ROAS. Implement multi-touch attribution (decaying or U-shaped models calibrated to product type), set up post-purchase survey questions, and design at least one geographic hold-out study within the first 90 days to establish an iROAS baseline. Data shows a typical 18% to 20% halo effect from media-buying affiliates on other channels. Your job is to measure and defend that lift to leadership.
  • Build executive dashboards tracking the metrics that actually matter: Revenue Per Ambassador (RPA), Earnings Per Click (EPC), Cost Per Acquisition (CPA), incremental CAC, partner-level contribution margin, and percent-incremental vs. cannibalized traffic by partner.
  • Own healthcare-affiliate compliance end-to-end: TCPA (text and call regulation as it applies to affiliate-generated leads), HIPAA (patient data handling in any affiliate or community context), LegitScript certification, FTC endorsement guides, FDA promotional rules, and platform-specific health-advertising restrictions across Meta, Google, TikTok, Reddit, and Quora. Vet every creator brief, every code, every landing page through this lens.
  • Partner with the internal content and creative teams to develop assets that work for community and affiliate audiences specifically, which is a different brief than what works for paid social.
  • Identify and test new partnership and community-driven growth opportunities as they emerge: TikTok Shop affiliate, performance-publisher marketplaces, brand-as-affiliate plays, card-linked offers (Amex, Chase Pay), and any other channel where the unit economics and compliance posture work for a cash-pay telehealth brand.

QUALIFICATIONS:

EDUCATION & CERTIFICATION:

  • Bachelor’s degree in marketing, communications, business, or a related field, or equivalent hands-on experience running affiliate programs at scale.

FUNCTIONAL COMPETENCIES, SKILLS, KNOWLEDGE & EXPERIENCE:

  • 3 to 5+ years owning an affiliate program end-to-end in DTC, with strong preference for cash-pay DTC subscription businesses, ideally telehealth (Hims, Ro, Nurx, 30 Madison / Remedy Meds, LifeMD, Cerebral, Done) or comparable subscription health/wellness, beauty, or supplements.
  • Hands-on platform experience with EverFlow strongly preferred. Impact, CJ, Partnerize, Tapfiliate, Trackdesk, or similar will be considered with EverFlow ramp-up baked into the first 30 days.
  • Demonstrated ability to negotiate every flavor of affiliate deal: flat-fee, CPA, CPC, revenue share, hybrid, and performance-based influencer/celebrity contracts.
  • Pre-existing relationships with tier-1 health-vertical publishers (Healthline, Hearst commerce, Condé Nast verticals, Good Housekeeping, Forbes Health) are a significant plus. Healthline in particular requires 6-figure minimum engagements. Candidates who have already navigated that conversation will be prioritized.
  • Working fluency in Reddit and Quora as marketing channels. You know what works, what gets you banned, and how to build authentic presence without setting off the community’s BS detector. This is a separate skillset from running paid ads and is worth flagging honestly on your resume.
  • Strong analytical skills: comfortable pulling data from affiliate dashboards, GA4, server-side conversion APIs, and post-purchase survey tooling. Bonus for hands-on time with Northbeam, Rockerbox, Triple Whale, or comparable internal attribution truth.
  • Deep, working knowledge of healthcare advertising compliance: TCPA, HIPAA, LegitScript, FTC endorsement guides, FDA promotional rules, and the specific health-ad restrictions of Meta, Google, TikTok, Reddit, and Quora.
  • Excellent written communication. You can write a publisher pitch that gets a response, a Reddit post that doesn’t read like a press release, an influencer brief that survives compliance review, and a weekly performance report that doesn’t bury the lede.
  • AI-fluent in the way that matters for this role: you use AI tools to accelerate publisher research, brief generation, fraud-pattern analysis, and creative variant testing, not as a research toy, but tied directly to RPA, EPC, and incremental CAC improvements.

LEADERSHIP & BEHAVIORAL COMPETENCIES:

  • This is an individual-contributor role with no direct reports to start. You will need to lead through influence: directing agency relationships and external partners, collaborating across CMO, content, creative, and clinical operations, and holding compliance vendors accountable.
  • Comfort operating inside a lean 12-person entity org where the President is one Slack message away and political layers are thin. You’ll work directly with senior leadership and your work will be visible immediately.
  • Disciplined communicator. You write performance reports that travel up to the PE board without translation, and you brief creators in plain language that holds up under FDA scrutiny.

PERSONAL CHARACTERISTICS:

  • High integrity. The compliance stakes in DTC telehealth affiliate are real, federal-exposure real, and we need someone who is paranoid about it on purpose.
  • Self-directed and hands-on. The President’s exact framing was “someone who wakes up thinking about affiliates and goes to bed thinking about them.” You do not need to be told what to do next.
  • Entrepreneurial. This is build-from-scratch work inside a recently acquired PE-backed company. Comfort with ambiguity and operational slack is required.
  • Resilient. The CMO tried this and couldn’t make it work. You’re walking in to a function that needs to be built from zero. Optimism with a thick skin is the right posture.
  • Honest under pressure. When something isn’t working, you’ll say so. When affiliate fraud is happening, you’ll surface it. When a publisher is brand-bidding, you’ll claw the commission back even when it’s awkward.
  • Strong written communicator across audiences: publishers, creators, Reddit moderators, PE leadership, and clinical operations.
  • Compliance-first mindset. In this category, compliance is the moat, not the obstacle.
